If you are unable to complete the New Patient Packet before your appointment please plan on arriving 30 minutes prior to your appointment time in order to complete the questionnaire in the office. Some 医生 will not see new patients without the completed forms which may necessitate rescheduling the appointment.

You should plan to spend about an hour and a half during your initial appointment. This includes arriving 15 minutes prior to your scheduled appointment time to complete registration. Your initial evaluation is your first meeting with our physician so please be prepared to provide information regarding previous treatments, 医生, 研究和测试. The doctor will discuss treatment options with you and if we are authorized for additional visits beyond the initial consultation, we will attempt to obtain authorization for these treatment recommendations. 重新安排

Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. Failure to provide sufficient notice may result in fees applied to your account. Treatment visits must be scheduled at least one time every twelve months or a new referral is required.
